I, too would like to hear more about "boothex". What is it, exactly?

Regarding the troubleshooting, it would be helpful to scope some of these signals. You should see be able to see the serial data and clock streams coming in from the floppy drive, when the head is at track 0, for a start. As Bill suggested, it also would not be a bad idea to check continuity of each wire in the ribbon cable. It may be easiest to do that with both ends plugged in, then probe the PCB pads with the power off.

Do you have a digital scope, or an analog one?

Hi Dave,

Thanks for the suggestions. I have a digital scope and will look for the signals and check the cable for continuity. The file I keep referring to was provide by Grant who generously packaged everything a person would need to build OS65D disk 5 from scratch, track-by-track. His instructions and the link to the image of disk 5 follow:

OK here they are (code necessary to build disk 5 track by track) http://klyball.com/OSI/33disk5.zip

Via a serial connection, the first file to load is the raw boot folder (BootHex)

Use word pad to open and select all and copy and paste into your connected terminal program after you press the L (you should see the address and data changing values)

After it loads it runs automatically you should see A*

Type INI (this will format the disk)
1. type HO (this will home the head back to track 0)
2. you can then reset system (don't remove power or you will need to reload the boot file)
3. enter M then L and load TX.hex starting with T1(TX=track#file) the same way, when finished loading you should see the A*
4. type SAVE 01,1=3300/8 (changing this to which ever track you are writing
5. repeat

The save format is TT,S=XXXX/P were TT= track, S= sector XXXX=memory location of file and P=pages to load

When you get to track six you need to do it 4 times starting with save 06,1=3300/1 then 06,2=3300/1 and so on
I labeled the multi sector tracks with the sector and page #

After you get them done we need to do track 0

Reset and load up T0
should be back to the A*
type HO
type CA 0200=39,1
type GO 0200

You should now have track write utility running
C4PDF w. 24K, 2 x 5.25 MPI B-51 drives
C1P RevB with fully-populated 610 board and A-13. Klyball D-13.
Spares: 600 Rev.D, 610 board, 540A, A-13 with homebuilt data separator, MPI B-51 logic board (w/o data sep module) and mechanicals
