Actually, I started with your schematic, and just re-arranged everything, then started swapping some stuff out. I have seen lots of multi-turn trimmers with the inline configuration, so I didn't really think much of it. I just checked the Mouser catalong, and there are several pots that will fit as-is.
I have gone through the design once more, and this time, I made sure that there is proper termination on all the lines, matching the OSI schematic for the output lines, and 100 ohm pullups for the inputs. I removed the pullup from the /STEP line, since it's terminated on the floppy controller. I also added a couple of additional jumpers, in case some drives require the DENSITY_SELECT line pulled low.
I've decided to name the board the OSIWEB CA-13x, to indicate it's purpose, and origin. I've uploaded the new zip file here. If this looks OK, I'll send it off.

I got the boards a while ago, but have been too busy with work to get my OSI systems up and running until last week. I'd like to make sure everything else works first, then check out the board. Nama has a board, and is building one up, so he may get to test it out before me. I would not be surprised if some fixes will be required--Murphy's law is alive and well.

I have attached a picture of the board. I need to find some 7438's and a 74121. I have already found a 74123 in my box of parts.
